All non-faculty/staff/student guests visiting the university in relation to Durham School activities (classes, research, events, etc.) must be reported for COVID-19 tracking purposes. This form must be completed at least 48 hours prior to your guest's visit. A COVID-19 policies and procedures document will be sent to your guest via email to sign before they arrive on campus. If your guest needs PPE, please indicate that using the option below.
