This form can be used by COE students on City, East, and Scott campuses. We encourage you to use this form if you have already tried to resolve this issue and it has not been resolved. You will receive an auto-confirmation immediately after submission. Protecting the privacy of students is extremely important to UNL COE. Information received is strictly confidential and will only be disclosed for the purpose of resolving this concern. You can expect an email or phone confirmation from the Associate Dean within 3 days to acknowledge receipt and outline next steps.

We understand that reporting a concern can sometimes be difficult. The university prohibits retaliatory conduct taken against any person who reports concerns, files a police report, or files a complaint with the university per Board of Regent policy RP-1.1.3. Measures will be taken to protect you from any retaliatory actions. In addition to the student feedback form reporting option, you can report using UNL’s TIPS Incident Reporting System.
